LPN Charge Nurse – Night Shift
Administer medications and treatments as prescribed by physicians, ensuring that residents receive appropriate and timely care.
Assist with resident assessments and develop care plans in collaboration with the interdisciplinary team.
Monitor and document residents' health status and response to treatments and report any changes or concerns to the RN or appropriate healthcare provider.
Coordinate with other healthcare providers, such as physicians, therapists, and social workers, to provide comprehensive care to residents.
Supervise and direct the work of nursing assistants and other healthcare providers, ensuring that care is delivered according to facility policies and procedures and in compliance with state and federal regulations.
Modify and review care plans to ensure appropriate delivery of cares after a change in condition.
Understand, follow and ensure safety and infection control policies and procedures are being appropriately followed.
Participate in quality improvement initiatives and ensure that care is delivered in a safe and effective manner.
Maintain accurate and complete records of resident care and communicate effectively with the interdisciplinary team, residents, and families.
